[clinical-curation] Author a MAFLD module (ALT/AST + obesity)

Source has ALT, AST, and we can infer obesity from BMI proxy. MAFLD is highly prevalent in TR. Need: (a) the ALT/AST cutoffs that TR clinicians actually act on, (b) typical workup (US? FibroScan?), (c) first-line guidance (lifestyle vs. pharma — vit E? GLP-1?). Implementation: src/syntha/modules/mafld.py.
