Release prep for 2.0.0. CHANGELOG only.

2.0.0 was never published: npm latest is 1.0.30, and there is no tag or GitHub release. So everything under Unreleased ships in it:

  • The entry is dated 2026-10-26, and its Added and Changed lists are combined. All 110 entries are kept.
  • Six bullets from the original 2.0.0 notes were reversed by later work in the same release. They now describe what ships:
    • exit 1 starts at high severity (--fail-on=low restores failing on everything)
    • the slop index replaces the 1.x score, and the upgrade notes point to slopIndex
    • the README credits the slop index idea
    • npm run build compiles with tsc

Reviewed with /review (reviewer on Haiku 5.5). The first commit failed on a stale exit-code line in the upgrade notes, which the second commit fixes. The second commit passed. Its three P3 wording notes are added to #109.

2.0.0 was never published, so everything under Unreleased ships in it.
The entry is dated today, its Added and Changed lists are combined,
and four of the original 2.0.0 bullets that later work in the same
release reversed now describe what ships: exit 1 starts at high
severity, the slop index replaces the 1.x score, the README credits
the slop index idea, and npm run build compiles with tsc.

The upgrade section still said exit 1 starts at medium severity and
that the slop score was gone. Exit 1 starts at high (--fail-on=low
restores failing on everything), and the 1.x score is replaced by
slopIndex in the JSON report, on a different scale.

Skipping CodeAnt AI review — this PR is a back-merge between long-lived branches (release/2.0.0 → main). The diff here has already been reviewed when the underlying commits landed on the source branch, so re-running analysis would produce duplicate findings on already-reviewed code.

If you want to analyze this anyway (e.g. you resolved conflicts with new logic), comment @codeant-ai : review and CodeAnt will start a review.

### Upgrading from 1.x
- Remove `severityOverrides` entries for the removed rule ids listed below. They are ignored now. The replacement ids are listed next to each removed rule.
- CI that fails on any finding should expect exit 1 only for high severity and above, or pass `--fail-on=low` to keep failing on everything. `--strict` still exits 2 on critical findings.

Clarify the --fail-on=low exception.

--fail-on=low fails on all rule findings except informational package-age findings. Replace “everything” with “all non-informational findings.”
