Fix four run frictions hit by real benchmark sessions
1. _build_model: default timeout=300 + num_retries=2 (overridable via
MODEL_KWARGS) — a hung API call previously froze the whole-repo session
forever; observed as a 43-min dead run with zero progress.
2. Incremental trajectory: TrajectoryWriter appends per agent step, so a live
run's progress is visible from outside (final save_trajectory write remains
authoritative, identical format). Previously the trajectory existed only
after session end, making hangs and healthy runs indistinguishable — and a
previous crash's leftover file impersonated the live run.
3. Preflight now also exercises the per-call cost accounting the agent runs
every step (LitellmModel._calculate_cost). A raw _query passes for models
missing from litellm's price map, but the real session then aborts on the
cost RuntimeError — preflight used to say 'safe to launch' right before it.
4. make run writes out/<stamp>/submission.json (+ trajectory alongside) so
successive runs no longer clobber each other's outputs (STAMP overridable).
Tests: 3 new _build_model tests, 4 TrajectoryWriter tests, 2 preflight
cost-tracking tests (+ updated fakes). Full unit suite green.
