security: quarantine Telegram Forwarder (ToS-risk tool) out of public tip

Wave 1.4 (master prompt): tools/ops/telegram_forwarder advertises
'bypass Restrict Saving Content via download-upload' (its README line 34
and the root README features list). Advertising a ToS-circumvention
technique inside the public repository of a MEDICAL project is a
reputation/legal liability regardless of technical legality.

Reversible quarantine (no history deletion):
- git rm -r tools/ops/telegram_forwarder — the tool remains fully
  recoverable from git history and from this branch itself
- README.md: the public feature section is removed
- CHANGELOG/git-sync references are historical/standalone-repo strings
  and were intentionally left untouched

Note: same-named standalone repo (telegram-tools) is separate and is NOT
touched by this PR; its data-flow documentation is tracked as Wave 2.4.
