ci: pr-autopilot approve workflow for hermes-exosphere's own PRs - #959
Adds .github/workflows/pr-autopilot-approve.yml, which lets pr-autopilot get hermes-exosphere's own PRs approved once it has reviewed them.
Why
pr-autopilot runs as hermes-exosphere, so no extra paid seat is needed. That account also opens Kiln's, canary-doctor's and issue-autopilot's PRs, and GitHub lets no one approve their own PR. For those PRs, pr-autopilot posts its review as a comment and starts this workflow. The workflow approves as github-actions[bot], which the org ruleset already counts. Dependabot's PRs are approved the same way today.
What it checks before approving
It approves only if all of these hold:
hermes-exosphere started the run;
the PR is hermes-exosphere's own, still open, and its head is the commit pr-autopilot reviewed;
hermes-exosphere posted a review on that commit carrying pr-autopilot's approve marker.
It never checks out the PR, merges nothing (a person still merges), and passes its inputs to the shell only through env vars. A later push dismisses the approval, as the ruleset already does.
PRs by members and Dependabot don't use this workflow: hermes approves those directly.
Needs
Settings → Actions → General → Allow GitHub Actions to create and approve pull requests. It is evidently already on wherever Dependabot PRs get bot-approved.
The pending-review issue is fixed, but the workflow remains able to approve arbitrary PRs when dispatched from a mutable branch ref. This is a blocking authorization bypass.
Changes
Adds a manually dispatched GitHub Actions workflow that validates a Hermes review and posts a PR approval.
Validation
PassedStatic inspection of .github/workflows/pr-autopilot-approve.yml — Reviewed all 78 added lines, including dispatch inputs, permissions, review selection, and approval submission. (0s)
SkippedGitHub Actions workflow execution — Requires GitHub-hosted workflow dispatch and a repository write-enabled GITHUB_TOKEN, unavailable in this harness. (0s)
Skippeddocker run --rm --network=none bash:5.2 bash -c '<shell probe>' — The nested-container probe could not start because the private Docker daemon had no local bash image and could not resolve the registry to fetch it. (11s)
Findings
High/High Dispatching a mutable ref bypasses approval validation — workflow_dispatch at line 28 may run the workflow file from a selected branch or tag ref, while line 39 grants that file pull-requests: write. A run started as hermes-exosphere can target a branch containing a modified copy that removes the actor, PR-owner, SHA, and review-marker checks, then use its write-capable github.token to approve any open PR. The checks are inside the same mutable workflow revision and therefore cannot enforce this boundary. (.github/workflows/pr-autopilot-approve.yml:28)

New Features
Added a manually triggered workflow that can approve eligible open pull requests when started by the designated account. It checks that the pull request belongs to that account and that the supplied commit matches its current head.
Approval requires the account’s latest submitted review on that commit to contain the required marker. The workflow exits without approving if its checks fail, and it does not merge pull requests.
Approves hermes-exosphere's own PRs as github-actions[bot] once
pr-autopilot has reviewed them; see the file for what it checks.

Walkthrough
A manually dispatched workflow validates the pull request number and reviewed SHA. It checks that the pull request is open, belongs to hermes-exosphere, and has the supplied head SHA. It then checks the latest matching review for the approval marker before submitting an approval.

Summary
Dispatch and workflow conditions .github/workflows/pr-autopilot-approve.yml
Documents the approval conditions and required Actions setting. Defines the required pull request number and reviewed SHA inputs, pull-request write permission, per-pull-request concurrency, run restriction, and five-minute timeout.
Validate and submit approval .github/workflows/pr-autopilot-approve.yml
Validates the inputs, pull request state, owner, and head SHA. It selects the latest submitted, non-pending review by hermes-exosphere on that commit and requires its body to contain the matching approval marker. It then submits an approval review. The workflow does not check out code or merge the pull request.

workflow_dispatch can run the workflow file from a selected ref. This workflow grants pull-requests: write and has no ref check. A writable branch can therefore remove the review checks and use github.token to approve another pull request.
Enforce the trusted ref outside this editable workflow, or use a dispatch design that cannot select an editable ref. A ref check inside this file is not sufficient.

Evidence: Lines 66-70 emit only submitted_at and the marker boolean, then use sort | tail -n 1. GitHub review timestamps have second precision. If an approving review and a later negative review are submitted in the same second, the tied records sort as ...\tfalse then ...\ttrue; tail -n 1 selects the older approval. The workflow then posts an approval despite its stated requirement that the latest review approve.
Required change: Select the newest review with a stable tie-breaker, such as the review ID, and require that selected review to contain the marker. Filter out unsubmitted reviews as part of that selection.

sorted last. Review ids only grow (found by pr-autopilot's review of

Dismiss an approval when the head changes during submission.
The head check and approval POST are separate operations. If a push occurs after line 64, stale-review dismissal runs before this workflow creates its approval. GitHub accepts the older commit_id and returns an APPROVED review. That review was not present when the push occurred, so the stale-approval rule does not dismiss it. It can therefore satisfy the required approval for the new head.

High: A pending draft review can be treated as an approval verdict
Evidence: The review query at lines 67-71 filters only login and commit ID. GitHub's pull-request review list includes pending reviews, and a pending review created with the approve marker has a higher review ID than an earlier submitted negative verdict. The query selects that draft and line 73 posts an approval even though the review was never submitted.
Required change: Restrict the query to submitted reviews, for example by requiring .submitted_at != null (or equivalently excluding PENDING) before sorting by review ID.

Hermes — High/High (SEC-001): Dispatching an editable ref bypasses the approval checks

A pending review's id predates its submission, so the highest id is not
always the latest verdict (found by pr-autopilot's review of
FailproofAI/pr-autopilot#2). Order by submitted_at, the id breaking
one-second ties, and skip unsubmitted reviews.
