chore(aw): upgrade gh-aw to v0.92.0, drop stale model pin, wait for app readiness

Recompile all agentic workflows on gh-aw v0.92.0. The auto model alias now
resolves for this org, so remove the gpt-5.6 pin from accessibility-review.
Add a readiness gate so Playwright only starts once the app is listening on
port 3000, and print the app log if startup times out.
