Includes several patches to the STL conversion operator, as well as some enhancements and increased control over parameters.
Geometry Defects
Marching-cubes vertices are passed to SimpleITK in reversed index order
Where:
STLConverter.convert, the vertex loopMechanism:
sitk.GetImageFromArraymaps numpy axes(a0, a1, a2)to ITK index(i, j, k) = (a2, a1, a0). The App SDK array is(depth, row, col), so a SimpleITKcontinuous index is
(col, row, depth)— the reverse of whatmarching_cubesreturns.Passing them through unreversed transposes the mesh and applies the in-plane spacing to the
slice axis and vice versa.
Observed impact: for anisotropic cases (i.e. in-plane spacing differs from slice spacing), the STL could be stretched or compressed along the in-place and slice axes. The degree of stretching or compression depends on the anisotropy ratio between the depth and column spacing. For example, testing with the MONAI-provided abdominal CT series (204 SOP instances) with DHW =
[1.5, 0.789, 0.789]mm, the output STL columns are stretched by 1.5/0.789 = 1.901 and depth is compressed by 0.789/1.5 = 0.526. This results in an elongated, flattened STL (also seen in Issue #389):On isotropic data the scaling error vanishes, but the axis swap does not. The mapping
(x, y, z) → (z, y, x)has determinant −1, so the mesh is mirrored. A mirrored organstill looks like an organ in a standalone STL viewer, which is likely why this went
unnoticed, but it does not register against the source images.
Fix: reverse the index order before the transform:
This aligns the channel spacing, resulting in a geometrically accurate STL:
Image origin is never set
Where:
SpatialImage._load_datacallsSetSpacingandSetDirection, but neverSetOrigin.Mechanism: the SimpleITK origin defaults to
(0, 0, 0), so every vertex is translated bythe first slice's
ImagePositionPatient.Impact: meshes are never in patient coordinates. A standalone STL viewed on its own looks
correct, but it will not overlay on the source series, and it cannot legitimately carry a
FrameOfReferenceUIDif wrapped as a DICOM Encapsulated STL. This is visible when loading in STLs produced from the previous version of the operator as a segmentation in 3D Slicer:Fix: take the translation from the affine already present in the image metadata.
STLs are now aligned with the source DICOM series in 3D Slicer:
ITK direction matrix is built from rows instead of columns
Where:
SpatialImage._load_dataMechanism: an ITK direction matrix holds the physical directions of the index axes as its
columns. Stacking the three cosine triplets as rows produces the transpose, which for a
rotation is its inverse.
Impact: none for canonical axial
ImageOrientationPatient, where the matrix is theidentity and D = Dᵀ. For an oblique series the mesh is rotated by twice the tilt angle, about
the volume-corner origin rather than about the organ.
Fix:
Note the DICOM naming asymmetry when assigning the triplets:
ImageOrientationPatient[0:3]is the "row direction", meaning the direction of travel along a row, i.e. of increasing
column index (ITK x).
PixelSpacing[0]("adjacent row spacing") is the step along therow index (ITK y). The two "row" attributes refer to different axes.
Spacing tuple order does not match the array axes
Where:
STLConverter.convertMechanism: the spacing tuple is
(row, col, depth)while the array is(depth, row, col); the isotropic-resample loop pairs them positionally.Impact: the wrong axis is resampled to isotropic — the slice axis is left anisotropic
while an in-plane axis is upsampled by the anisotropy ratio for no benefit. Wasted memory and
runtime, and a lower-quality mesh. Final vertex coordinates are unaffected, since they are
rescaled back into source index space before the physical transform.
Fix: carry a spacing tuple ordered to match the array,
(depth, row, col).SetSpacinghas the same issue and should be
[col_ps, row_ps, depth_ps]in ITK(x, y, z)order,though this is a no-op wherever
PixelSpacing[0] == PixelSpacing[1].Class-Selection Defects
Class-filtered mask was being discarded
Where:
STLConverter.convertnew_ndais rebound to a resize of the unfiltered label array on the line after it isbuilt.
Impact:
class_idshas no effect.marching_cubes(level=0.5)on an integer label mapfinds the outer boundary of every non-zero label, so everything gets meshed. This is
masked in the bundled single-label example, where
keep_largest_connected_component=Truehas already binarised the array.
Fix:
resize_volume(new_nda, ...).Connected-component filter runs before the class filter
Where:
STLConverter.convertMechanism:
get_largest_ccreturns a 0/1 array, sonda == class_idis uniformly Falsefor any
class_id > 1.Impact, once B1 is fixed: any
class_idsother than 1 produces an empty mask andmarching_cubesraisesValueError: Surface level must be within volume data range, killing the fragment.
class_ids=[1]"succeeds" but returns the largest connected componentof the whole label map relabelled to 1 — on abdominal CT that is whichever organ is largest,
not label 1.
Additionally,
skimage.measure.labelgroups by equal value, so calling it on a multi-labelarray produces per-organ components rather than a binary component analysis.
Fix: filter to the requested classes first, then apply the CC filter per class, then
union. Applying it to the union is a distinct behaviour that silently discards organs.

step_size5 → 1, andallow_degenerate=False. A stride of 5 on the isotropic griddiscards small and thin structures and leaves degenerate triangles, so the mesh is often
not watertight and its enclosed volume is not meaningful. This meaningfully increases
output size; callers who prefer the old density can pass
step_size=5. Noteallow_degenerate=Falseis not exposed as a parameter — degenerate triangles are thereason the enclosed volume was previously unusable, so there is no reason to opt back in.
smooths the entire acquisition to mesh one organ. A margin of ≥1 source voxel is required

before any resampling or smoothing, exposed as an attribute. The mesh volume should never
be used as the measurement — it is affected by binarisation, resampling, the 0.5 threshold
and smoothing — but logging both gives a useful QC signal.
organ continues outside the scan and any volume derived from it is a lower bound. Exposed

fail_on_emptyflag preserves the raising behaviour for callers that want it (now

write_stlis retained as a standalone helper, sonumpy-stlremains an optionaldependency but is no longer used by
convert.Validation
For our CCHMC Liver-Spleen segmentation model, liver and spleen STLs were rasterised back onto the grid of the DICOM SEG it came from and compared voxel-for-voxel. To create analogous meshes to the DICOM SEGs, the following parameters were used to remove any smoothing:
The mesh is transformed into the SEG's voxel-index space first, so the target grid becomes a unit-spaced axis-aligned lattice and all oblique geometry is carried by the affine — nothing is resampled, and the comparison adds no interpolation error of its own.
Exactness is the correct expectation for an unsmoothed mesh: linear interpolation of a binary mask crosses 0.5 at voxel faces, and marching-cubes corner chamfering cuts through face midpoints, so neither ever crosses a voxel centre — which is where the rasterisation samples.
This test set include an oblique case (
ImageOrientationPatienttilted 2.046°), which produce perfect DICE, RVE %, and no offset.Summary by CodeRabbit
