[WIP] Agentic/MCP skill wrapper pattern for MONAI Deploy operators - #590
kyounis-im wants to merge 2 commits into
Conversation
Proposes and demonstrates the general pattern for exposing MONAI Deploy operators as agent/MCP-callable tools, discussed in the MONAI Deploy WG call: an AgentOperator-style skill wrapper (SKILL.md + skill_manifest.yaml) following the same convention as NVIDIA's internal medical-AI-skills repo, paired with a deterministic verifier skill (agent_output_quality_v1) that grades emitted evidence packs for schema validity, physical plausibility, and reproducibility without re-running inference. Runs in real mode (SegResNet forward pass on GPU/CPU) or --mock simulation mode with no GPU/MONAI dependency, matching the project's current CI, which has no GPU runner available. Data governance: no PHI is read, stored, or transmitted. Operates on a synthetic volume or caller-supplied de-identified/public data only. See SKILL.md 'Data governance' section for the on-prem vs public-dataset deployment modes this does and does not address. Signed-off-by: Khaled Younis <kh.younis@gmail.com>
WalkthroughAdds a MONAI lung-nodule agent skill with real and deterministic mock execution, a structured evidence-pack manifest, a five-gate verifier, usage documentation, and smoke tests. ChangesLung-nodule agent skill

Merge Risk: 🟡 Moderate · up to This change adds a self-contained reference skill and does not alter existing SDK behavior, but the skill's declared manifest cannot be parsed as written, its documented input contract advertises volume paths the runner does not accept, the sensitivity threshold has no effect on results, and the paired verifier's reproducibility check can report success without actually re-running the skill. These should be corrected before merge so the reference pattern behaves as documented. 🚥 Pre-merge checks | ✅ 4 | ❌ 1❌ Failed checks (1 warning)

black and isort auto-fixes applied cleanly. One flake8 B907 suggestion (use !r instead of manual quotes) is intentionally suppressed with noqa: verified the !r/repr suggestion produces single-quoted strings that Windows cmd.exe does not treat as quoting, breaking the shell=True reproducibility-gate command on Windows. Kept explicit double quotes with an explanatory comment instead. pytest, black --check, isort --check, flake8, and mypy all pass clean. Signed-off-by: Khaled Younis <kh.younis@gmail.com>

Implement the declared sensitivity_threshold behavior.
The CLI accepts any float, including values outside [0.0, 1.0]. Both execution paths only report this value in metadata. They do not use it to filter or classify findings. Therefore, threshold changes do not change the emitted findings.
Validate the range. Apply the threshold before emitting findings, or remove this input from the contract.

Remove the unsupported volume-input contract.
The manifest defines only study_id. The runner accepts only --study-id and always uses a synthetic tensor. It does not accept a DICOM study input, a NIfTI path, or a CT volume path.
Document study_id as the only current input. Add volume-path support before advertising it.

Apply the manifest runtime bounds to real mode only.
The manifest declares 0.05–5.0 seconds as the real-mode runtime bound. gate_runtime currently accepts inference_time_ms: 0 and does not check execution_metadata.mode. Reject values below 50 ms or above 5000 ms for real_monai results, and skip this bound for simulated_monai results. The mock path emits 340 ms, so this preserves the smoke tests.

Read an output produced by the rerun.
The verifier rereads the original result_path after it executes rerun_cmd. If the command writes result_json.json in another directory, the comparison reads the unchanged original file twice and reports PASS. The documented rerun command omits --output-dir, so it triggers this false positive.
Run the rerun in an isolated output directory and compare the result file created there.

Quick WG update, tying this back to a couple of open threads:
Happy to put up PRs for either once there's a preferred fix direction. |

Summary
Follow-up from the MONAI Deploy WG call where this was discussed live — opening as a draft PR early per CONTRIBUTING.md's guidance, rather than polishing in isolation.
Proposes and demonstrates the general pattern for exposing MONAI Deploy operators as agent/MCP-callable tools: instead of reading operator source, notebooks, and manuals, an agent can discover and invoke an operator directly. Follows the same skill-package convention as NVIDIA's internal
medical-AI-skillsrepo (SKILL.md+skill_manifest.yaml+ a paired deterministic verifier skill), applied here to a MONAI Deploy operator instead of a standalone model bundle. What's included

Raised on the WG call: where does data come from, who hosts, who provides GPUs, and any data leaving an institution requires a legal/security review even when de-identified. This wrapper doesn't change where inference runs or where data goes — it changes how an already-permitted operator is invoked. Two supported deployment modes: local/on-prem (MCP server + inference inside the institution's network, same boundary as any MAP today) and public-dataset/research (hosted, de-identified data only, e.g. TCIA). A real clinical PHI use case is a separate, harder problem this PR does not attempt to solve — see
SKILL.md"Data governance" for the full writeup.Status
Draft — opening early for discussion per CONTRIBUTING.md. Not yet run through flake8/black/isort/mypy locally; will do before marking ready for review. Looking for feedback on whether
skills/at the repo root is the right location, and whether theAgentOperator/ToolRegistrylayer should additionally be exposed as first-class SDK types (monai.deploy.agent) rather than living only inside the skill script, per the original architecture discussion.Summary by CodeRabbit
