What

New example app under examples/apps/cchmc-tcv-fast/: a MONAI Deploy App Package (MAP) that predicts total cardiac and lung volumes on non-contrast chest CT using an nnU-Net 3d_fullres 5-fold ensemble.

  • Output labels: 0 = background, 1 = heart, 2 = lung
  • Two run modes:
    • batch — monai-deploy run (one study per invocation, CLI)
    • serve — long-lived gRPC daemon with DICOM-series ingest (folder-pointing client)
  • Inference is a 5-fold ensemble mean on GPU, followed by post-processing and resampling of the label map to the input's native space.

Model weights

  • The 5.3 GB weight bundle is intentionally not in git. It is hosted on Zenodo:
    record 23391210, DOI 10.5281/zenodo.23391210, license CC-BY-4.0.
  • scripts/download_models.sh downloads and extracts the bundle into the gitignored models/ directory, then verifies every file against MANIFEST.sha256.

Notes

  • my_app/requirements.txt pins holoscan-cu13>=4.3.0,<4.7.0 to match the validated in-container stack (CUDA 13 wheels of holoscan are on PyPI).
  • Development uses a vendored editable nnunetv2; the MAP image installs nnunetv2==2.8.1 from PyPI.
  • CPU-only unit tests (scripts/test_*.py) run without a GPU; end-to-end validation requires a CUDA device.

Testing / verification

  • Repo checks (isort / black / flake8) pass for the new code.
  • CPU tests: scripts/test_emit_npy.py, scripts/test_inference_params.py, scripts/test_contract_synthetic.py.
  • Full in-container end-to-end (batch + serve, pixel-identical outputs) was validated on a workstation with a discrete GPU; it is not part of repo CI.

Summary by CodeRabbit

  • New Features
    • Added a cardiac and lung segmentation app that processes DICOM studies and produces DICOM SEG, SR, and SC outputs.
    • Added batch processing and a persistent service with health checks, study submission, processing-time reporting, and queue-capacity feedback.
    • Added tools to download and verify model files, submit individual or folder-based studies, and check generated outputs.
    • Added input validation and GPU memory checks to identify studies that cannot be processed.
  • Documentation
    • Added setup requirements, usage instructions, performance information, and operational guidance for the app and service.

Replace the deprecated cm.get_cmap with the colormap registry.

Matplotlib 3.7 deprecated matplotlib.cm.get_cmap. Matplotlib 3.9.0 removed it, and 3.9.1 added it back with a deprecation and a planned removal in 3.11. The pinned matplotlib==3.10.9 still has the function. If the pin moves to 3.11, this module-level call raises AttributeError at import time. operators/__init__.py imports this module, so the whole operator package would then fail to import.

Call get_all_series() before taking its length.

When a selection contains conditions, _select_series evaluates this log statement. study.get_all_series is a bound method, so len(study.get_all_series) raises TypeError before series selection completes. Both batch and serve configure Sample_Rules_Text.

Handle publish_study errors and set the OK response only after publish succeeds.

The module docstring says finalize is defensive and does not raise after the study slot is handed over. The code does not do this:

  • publish_study raises OSError explicitly when the stage directory is missing. final.mkdir and os.rename can also raise, for example on permission errors, ENOSPC, or a non-directory at final/SUB.
  • There is no try/except around the publish call.
  • entry.response is already set to OK at Line 214, before publish runs.

When publish raises, purge_study_scratch, study_context_clear, STATE.end_study, and done_event.set are all skipped. The exception leaves compute, and in holoscan 4.2 that stops the graph. The RPC hangs until DEADLINE_EXCEEDED, and the daemon restarts or stops responding. One study with an unwritable output path is enough to cause this.

Publish first. If publish fails, record the failure and use the typed INTERNAL path. Run the release steps in finally.

Make the thread pool larger than the number of ProcessStudy calls that can block.

Each ProcessStudy handler holds a pool thread for the whole study (entry.done_event.wait, up to 900 s). Admission allows 1 study in flight plus queue_capacity (default 4) queued studies, so up to 5 handlers can block at once. The pool has 4 workers. With 4 ProcessStudy calls in progress, every worker is occupied, and CheckHealth waits in the server queue without an answer. This breaks the proto contract "Must answer fast even while a study is BUSY". A liveness probe can then time out, and the orchestrator can restart a healthy daemon. The same limit also makes the typed RESOURCE_EXHAUSTED queue-full rejection unreachable: excess calls wait in the gRPC server queue and never reach the handler.

Size the pool from depth. Keep spare workers for CheckHealth and for rejection replies.

Reject an output path that is the payloads root or overlaps the input path.

_validate checks only that p_out is under root. It accepts p_out == root, p_out == p_in, a p_out that contains p_in, and a p_out inside p_in. On any study failure, EgressOperator._finalize (egress_operator.py Line 227) and _finalize_entry (Line 607) run shutil.rmtree(ctx.output_path). Example trigger: a request with output_path=MAP_PAYLOADS_ROOT, or with output_path equal to input_path, followed by any pipeline failure. The daemon then deletes every study's inputs and outputs under the root, or the caller's input DICOM. Any client that can reach the unauthenticated port can send such a request.

Reject these cases in _validate. As a second safeguard, make the failure cleanup remove only the SEG/SR/SC subdirectories, not the whole output tree.

Restrict study_uid to safe characters before using it in filesystem paths.

study_uid is only compared with the on-disk DICOM StudyID. StudyID is an SH value, and SH allows /. The UID then goes into these paths:

  • stage_dir_for builds output_path.name + ".tmp-" + study_uid (serve_publish.py Line 77).
  • scratch_dir_for builds study_{uid} (serve_scratch.py Line 79).

A UID such as x/../../.. produces paths outside the payloads root and outside MAP_SCRATCH_ROOT. purge_stale_stage, discard_stage, create_study_scratch, and purge_study_scratch all run rmtree on those paths. The attack needs two things: write access under the payloads root, and access to the unauthenticated gRPC port. With both, a client can delete directories outside both roots.

Reject UIDs that are not a plain path component.

Escape the pipes in the SR table row.

The formula |det(affine)| contains literal | characters, so Markdown reads them as column separators. The row then has 5 cells while the header has 3. As a result, the rendered table drops the formula text (markdownlint MD056). Escape the pipes or reword the formula.

Align the documented volume precision with the implementation.

calculate_volume_ml converts the rounded value to int, so the SR contains whole-milliliter values. The checker uses the same conversion. The README currently states that the value has two decimal places. Update the README to document whole-milliliter output, or change both the producer and checker if two-decimal output is required.

Skip this test when the model bundle is absent.

models/ is gitignored, and you must run download_models.sh (a 5.3 GB download) to get it. In a fresh checkout or a CI run, load_inference_params(MODELS) therefore raises FileNotFoundError. Run the child processes with sys.executable and keep the parent environment.

The test sets env = {"PYTHONPATH": ...}, which removes PATH. CPython then looks up "python" in os.defpath (/bin:/usr/bin) instead of the active venv. The import smoke check then runs with the system Python, which usually lacks grpc, numpy, and the other dependencies. As a result, the check fails, or it tests the wrong interpreter. VENV_PY = "python" causes the same problem in check_router. Use sys.executable and copy os.environ.

Publish the complete study as one unit.

If a process stops after one rename, final contains only part of the new study. On a re-submit, this loop can also combine new subdirectories with old ones. Removing an old subdirectory before its rename creates another gap if the rename fails. Use a publication strategy that exposes either the prior complete output or the new complete output.

Convert disk-usage errors to ScratchError.

If MAP_SCRATCH_ROOT is missing or unavailable, shutil.disk_usage raises OSError before the mkdir error handler runs. Ingress catches ScratchError, so this case bypasses its typed RESOURCE_EXHAUSTED rejection. Catch the capacity-check error and raise ScratchError with the root and cause.

Call name.capitalize() when building SR volume lines.

The current code inserts the bound method instead of the structure name. The resulting SR lines do not match VOLUME_LINE_RE, so the SR format and exact-volume checks fail.

Report an empty input directory as a usage error instead of crashing.

If inp contains no *.dcm files, n_slices is 0. Then _decode_seg_masks raises ZeroDivisionError at f // n_slices. If that call is skipped, in_dir_files[0] raises IndexError at Line 215. Require the legacy Holoscan CLI for this inspection.

If an operator installs my_app/requirements.txt in the inspection environment, its holoscan-cli>=4.3.0 requirement can select a release without the legacy holoscan_cli.packager interface. The import then fails before any check runs. Pin Holoscan CLI 4.2.0 in the documented inspection environment, or reject incompatible versions with a clear preflight error. (github.com)

Replace the per-pixel Python loops with a numpy reduction.

The callers use the decode only to answer one question: does each segment have at least one foreground voxel? The loops still visit every pixel of every frame. For the 480-slice study that is 3 × 480 × 512 × 512 ≈ 377M Python iterations, so the check takes minutes. A numpy reduction per segment gives the same answer in well under a second.

detect_available_folds crashes on a fold_all directory.

nnU-Net creates fold_all when a model is trained on all data. If such a folder holds the checkpoint, int(d.name.split("_")[-1]) raises ValueError. That error escapes load_inference_params, and the model load fails. The module says it is driven by configuration, so a standard nnU-Net layout should not crash it. Skip suffixes that are not numbers, or support "all" explicitly.

Proposed fix

Same-UID calls can delete the running study's scratch and staging directories.

The pending-UID check at Line 248 and the work after it (create_study_scratch, purge_stale_stage) do not run in one critical section. Two triggers exist:

  • A same-UID call is admitted after this call passed step 3 but before Line 248. _uid_pending is then True, and the code skips scratch creation. Line 262 still calls purge_stale_stage without a condition. If the output path is the same, this removes the running study's <out>.tmp-<uid> stage while the writers use it.
  • Two calls both read _uid_pending == False. One is admitted and starts work. The other then calls create_study_scratch, which runs rmtree on the running study's study_<uid> scratch.

The rejected call then fails with ALREADY_EXISTS at step 5, but the files are already gone. The running study then fails in mid-pipeline: SC overlay temp missing, or publish_study raising OSError("stage dir missing") in Egress _finalize.

Reserve the UID under the lock at step 3. Release the reservation on every rejection path. With the reservation, no second call can reach the destructive steps.

Proposed fix
             with self._ingress._lock:
                 if ctx.study_uid in self._ingress._pending_uids:
                     _abort(...)

+ self._ingress._pending_uids.add(ctx.study_uid)
+ reserved = True

Then drop the _uid_pending branch. Skip the duplicate check in step 5 for this call's own reservation. In finally, run _pending_uids.discard(uid) when reserved and not admitted.

Add a flat-import fallback for load_torch_checkpoint.

In the flat app layout, build_network_from_params() can fail at model setup because it imports load_torch_checkpoint only as my_app.config. The batch and serve paths both use SlideWindowOperator, so startup can fail before checkpoint loading.

Move load_torch_checkpoint into the existing package-style and flat-import blocks, then remove the local import.

Use a private temporary path for the archive.

/tmp/tcv-model-bundle.zip is predictable. A second download can replace it while the first invocation extracts it. A local user can also pre-create a symlink at that path before a privileged invocation. Use mktemp -d and remove the private directory with an exit trap. The wget, unzip, and cleanup commands must use that path too.

Update the archive handling in the download script to create a private directory
with mktemp -d, save the curl download there, and register an exit trap to
remove the directory. Make the wget, unzip, and cleanup commands use the same
private path instead of the predictable /tmp archive path.

Verify the bundle before replacing the installed models.

If extraction or checksum verification fails, this sequence has already deleted the working models/ directory. It also extracts unverified archive entries into APP_DIR, outside the intended model directory. Extract into a private staging directory, verify the expected files there, and replace models/ only after verification succeeds. Based on learnings, verify external artifacts with a cryptographic hash before using them.

Avoid nesting an alternate archive root under models/.

If the archive contains both an alternate root such as bundle/3d_fullres and any models/ entries, unzip recreates MODELS_DIR after line 42. The condition at line 47 still selects bundle/3d_fullres, and line 50 then moves bundle into the existing models/, producing models/bundle/3d_fullres. Move the candidate root’s contents into MODELS_DIR while preserving any existing entries.

Keep preprocess_reference on the pure-CPU path.

When resampling is required and separate-z resampling is disabled, the default HOLOSCAN_GPU_RESAMPLE setting sends preprocess_reference through stock_gpu_resize, which returns a CuPy array. This contradicts its pure-CPU fallback contract and can fail for NumPy-only callers.

Add an allow_gpu parameter and disable it in preprocess_reference.

🎯 Functional Correctness | 🟡 Minor | ⚡ Quick win

Fix the per-channel mask indexing in preprocess_reference.

norm_mask = nonzero_mask[slicer][None] has shape (1, X, Y, Z). norm_mask[c][0] therefore selects the first X-slice, which has shape (Y, Z), not the 3D mask. _normalize_channel then evaluates image[m] with a 3D image and a 2D boolean mask. This raises an IndexError when the shapes differ, or normalizes the wrong voxels when they happen to match. When there is more than one channel, norm_mask[c] for c >= 1 also raises IndexError. The bug is reached for any config with ZScoreNormalization and use_mask_for_norm=True. The GPU path at Line 874 uses the correct 3D mask.

Replace the per-pixel Python loops with NumPy reductions.

The decode loop visits every pixel in Python. For a 512×512 SEG with 3 × 300 frames, that is about 236 million iterations. The script can take minutes for this one check. The callers only test whether each segment has any foreground voxel. A NumPy any over the frame block gives the same answer in milliseconds.

🩺 Stability & Availability | 🟡 Minor | ⚡ Quick win

Fall back to the flat estimate on any estimator failure, not only on StudyShapeError.

Today only StudyShapeError triggers the fallback. estimate_study_peak_mib can raise other exceptions that skip it:

  • Line 194 in study_geometry raises IndexError when PixelSpacing has one value.
  • Line 168 raises KeyError when Columns is missing.
  • load_calibration raises FileNotFoundError, JSONDecodeError, or KeyError when the file is missing or malformed. A bad MAP_VRAM_CALIBRATION override is one example.
  • load_preprocess_params raises KeyError or FileNotFoundError.

Each of these reaches the catch-all at Line 408 and aborts the RPC as UNKNOWN. The scratch directory created at Line 253 is not purged on that path. A bad calibration path fails every request in this way. The StudyShapeError docstring requires that an estimation failure never hard-fails an admissible study.

Scope VRAM geometry to the selected DICOM series.

If a serve input contains multiple matching CT series, study_geometry counts image instances from every series under ctx.input_path and pools their z positions. The selector allows all matches, but DICOMSeriesToVolumeOperator converts only the first selected series. The estimator can therefore overestimate or miscompute the inference shape and reject the study with RESOURCE_EXHAUSTED.

Enforce one series per serve input, or make the estimator use the same selected series as the pipeline. At minimum, group headers by SeriesInstanceUID and estimate the largest matching series.

Use a collision-free staging name for each StudyInstanceUID.

_siu_short keeps only the final 12 characters. Distinct StudyInstanceUID values with the same suffix therefore select the same staging directory and out_ directory. The second study can reuse the first study’s input and output paths. Use the full validated UID or a collision-resistant digest for both names.

Do not treat a nonempty staging directory as complete.

If a copy fails after the first file, the next run returns that partial directory without copying the remaining files. The same behavior also reuses stale files when a study is restaged with changed inputs. Complete each copy in a fresh directory and publish it only after all files arrive, or validate the staged contents before reuse.

Preserve every DICOM instance when staging multiple series.

If two series contain a file named IM0001.dcm, the second copy2 overwrites the first in the flattened staging directory. The daemon then receives an incomplete study. Preserve series-relative paths, or assign a unique destination name to each instance.

Translate host staging paths to container paths before submission.

If Docker mounts /data/payloads at /payloads and the client uses --staging /data/payloads, this request sends /data/payloads/.... Store retained study logs in a private directory.

The fixed directory under the shared system temp directory is created without private permissions, and the client never removes its logs. With a common 022 umask, other local users can read the final summary containing StudyInstanceUID and StudyID values. Create a private 0700 directory and define when these logs are removed.

Vectorize the SEG decode loop.

The inner for y / for x loops run in pure Python for every frame. A real CT of 512x512 with about 300 slices has about 900 frames. That is more than 200 million iterations, so the checker takes minutes on the real studies it is meant to validate. Each loop only ORs every row into one per-column flag, so numpy can compute the same result.

The _decode_seg_masks docstring also says the result is "(H, W)" per slice. The code stores one width-sized column projection per slice. Update the docstring to match. The fallback branch at lines 132-137 has the same loop shape. Apply the same change there if that path matters.

Align the generated-code marker with the supported protobuf runtime.

TARGET_PB_VERSION writes a 6.33.0 marker, but my_app/requirements.txt permits protobuf 6.30.x. With that permitted runtime, importing the committed map_service_pb2.py raises VersionError before batch or serve mode can use the stub. Raise the runtime minimum to 6.33.0 if the generated code requires it. Otherwise, generate and verify stubs for the declared minimum runtime rather than changing only their version markers. Provide grpcio-tools to the dry-package check.

dry_package_inspect.py runs the staged codegen.py --check with sys.executable. The check calls regenerate(), which invokes grpc_tools.protoc. Since my_app/requirements.txt deliberately omits grpcio-tools, an inspection interpreter set up with only those requirements can fail before comparing the stubs. Provide the tool in the check environment, or change the check so it does not regenerate the stubs. Keep the tool out of the image’s runtime requirements.

Use the study slug for the .prev- path in purge_stale_stage.

stage_dir_for and publish_study both use study_slug(uid) for the staging and .prev- names. This line uses the raw study_uid instead. Two failures follow:

  • If the StudyID contains a space or an underscore, the purge targets a different path than the one publish_study creates. A stale .prev- tree is never purged. The next os.rename(final, prev) in publish_study fails on a non-empty directory. The study then fails with publish_failed.
  • If the StudyID contains /, Path.with_name raises ValueError. The admission handler aborts with UNKNOWN, and the scratch directory created earlier is not purged.

- output_path.with_name(output_path.name + ".prev-" + study_uid),

Fail closed when the checkout manifest is absent.

The app commits MANIFEST.sha256, so normal checkouts use its independent hashes. If that file is missing, this branch takes the manifest from the same archive as the models; an altered archive can update both and pass sha256sum --check. Reject that fallback or require a separately trusted digest.

L132:
L133: MONAI versions before 1.6.0 contain a remote code execution vulnerability in the algo_from_pickle() function due to unsafe pickle.loads() deserialization in monai/auto3dseg/utils.py. Attackers can craft malicious pickle files that execute arbitrary system commands when deserialized by the vulnerable function.

Record a decision for the known-affected monai==1.5.2 pin.

monai==1.5.2 is in the range that the cited advisories list as affected. The advisories list 1.6.0 as the fixed version. The pin's comment explains why 1.5.2 was chosen (numpy 2.x support, Python 3.12, numpy-2 code cleanliness), but it does not mention the advisories. The app code does not import algo_from_pickle, NumpyReader, or monai.auto3dseg, so direct exposure from app code is not established. Transitive use through monai.deploy SDK code is not checked.

Before raising the pin, confirm that monai==1.6.0 resolves with numpy==2.2.6 and torch==2.13.0+cu130. If it does, raise the pin and retest the image. If it does not, add a comment next to the pin that records the accepted risk and the reason for it.

Check whether monai==1.6.0 resolves with numpy==2.2.6 and torch==2.13.0+cu130;
if it does, update the monai pin and retest the image. If it does not, keep
monai==1.5.2 and add a nearby comment recording the known advisory risk and the
reason for accepting it.

Make the default startup sweep able to find prior scratch roots.

When MAP_SCRATCH_ROOT is unset, serve_scratch.scratch_root() creates a new temporary root for each process. After a hard kill and restart on the same filesystem, this sweep scans the new root. It cannot remove the prior process's study_* directories. Repeated restarts can retain study data and consume disk space. Use a stable private root for serve mode, or provide a safe way to find and purge prior process roots.

108:
Update the startup path around scratch_root() so serve mode uses a stable
private scratch root across process restarts when MAP_SCRATCH_ROOT is unset.
Preserve the configured-root behavior when the variable is set, and ensure the
startup sweep can find prior study_* directories in the stable root.

Serialize concurrent model installation.

Overlapping invocations can leave one complete bundle under models/ and move the other staged bundle into models/models/. mv reports success, and models/3d_fullres remains available, but the installation contains an unintended duplicate model tree.

The README documents a manual download command, and no CI invocation was found. This is a narrow setup-script race, so major severity is not warranted. Use minor severity. Hold a lock across the backup, install, and cleanup steps to prevent the duplicate tree.

Serialize the installation sequence in the script around the backup and staged
`mv` operations: acquire a lock before moving `MODELS_DIR` to `OLD_DIR`, and
hold it through installing `${STAGE_NEW}/models` and cleanup. Release the lock
on every exit path so concurrent invocations cannot interleave these steps.
