New-source form asks for the dated quote, the credit line and publisher status

Adds a required "Licence quote and date" field, a credit-required dropdown,
the credit line as worded by the source, and an "I publish this source"
checkbox; the issue chooser links to the open re-verify batch and to private
vulnerability reporting.

Closes #149
