fix: bundle plugin APIs as hard deps, fix npm video embed

- Move @opencode-ai/plugin + @opencode/plugin to dependencies so the
  dual v1(server)/v2(setup) entrypoint loads with either host alone.
  Optional peers + static imports caused ERR_MODULE_NOT_FOUND.
- Replace bare user-attachments video URL with markdown link (npm
  does not embed video).
- Normalize repository.url (npm pkg fix).
