Formatting issues in Formatting docs

I stumbled upon a few formatting issues related to how the markdown on GitHub is ingested into the actual https://make.wordpress.org/docs site.

There are probably more, but here are a few.

Note

While the callout design has been updated recently, the functionality is the same as the previous design, so this does not appear to be a new issue.

I've opened a PR that addresses the formatting parser issues reported here.

What was fixed

The markdown renders incorrectly on make.wordpress.org/docs when:

• Code fences are nested inside notice shortcodes ([tip], [warning], etc.)
• HTML tags are wrapped in backticks (e.g. <figcaption>, <img>)
• Non-breaking spaces use backticks around   in units-of-measurement examples

The PR also includes similar fixes for related pages tracked in #12 (procedures, tables, heading targets, image links, code examples, placeholders).
