Refine coarse candidates directly into ID and optional score buffers for supported contiguous Flat references. Reuse the existing legacy and Turbo distance kernels and preserve score ordering and normalization. Retain the generic refiner for unsupported requests and storage layouts. When direct refinement falls back after coarse search, reuse its candidates instead of repeating the coarse search. Add coverage for direct output, concurrent queries, storage modes and single-coarse-search behavior across late fallback cases.
