Add GPLv3 LICENSE and SPDX headers

Add the full GNU GPL v3 LICENSE file and update README.md with a License section referencing LICENSE. Add SPDX-License-Identifier: GPL-3.0-only headers to app.js, index.html, and style.css, and apply minor whitespace/newline cleanup across files.
