Refactor UI for multi-group class calendar generation

Redesigns the form to support multiple groups, each with its own schedule and class name. Adds dynamic group and schedule row management, and separates PDF generation into two buttons: one for a multi-group matrix and one for detailed per-group calendars. Improves data collection and PDF output logic to handle multiple groups and schedules.
