fix(ci): simplify copilot-setup-steps to minimal working config

Reduced to bare minimum for GitHub Copilot Workspace:
- Remove extra permissions (only contents:read needed)
- Remove cache cleaning (handled by actions)
- Use stable action versions (@v4, @v5)
- Minimal verification step
- Remove verbose comments

The 'Failed to launch agent' error suggests the workflow was too complex
or had permission issues. This minimal config should work reliably.
