Subject: [PATCH] docs: remove stale duplicate root ISSUE_TEMPLATE.md

GitHub only reads .github/ISSUE_TEMPLATE.md for the classic issue
template; the root-level ISSUE_TEMPLATE.md is unused by GitHub and had
drifted out of sync (stale wording, missing the performance-info line,
a typo: 'provide a the steps'). Remove the dead duplicate to avoid
future edits going to the wrong copy.
