VS Code 0.3.1 bump + drop Open VSX publishing - #14

We don't publish to Open VSX, and the OVSX_PAT step was failing the release with "Invalid access token". Remove the Open VSX publish step and its token env; releases now publish to the Visual Studio Marketplace only. Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>
Align the VS Code version with JetBrains (0.3.1) and document this release: "Context sent to AI" preview, "Thought for Ns" thinking panel, file-based skills, and the composer UX (Enter-to-send, slash palette). Co-Authored-By: Claude Opus 4.8 <noreply@anthropic.com>

Follow-up to #13 (which merged the Skills + transparency work up to the test fixes). These two commits landed just after that merge:
ci(vscode)— remove the Open VSX publish step (it was failing releases with "Invalid access token"); VS Code releases now go to the Visual Studio Marketplace only.chore(vscode)— bump the VS Code extension to 0.3.1 (matching JetBrains) + changelog for the transparency/UX release. No source/runtime changes beyond version + release CI.
