Closes #77

Remove theme cycle button from sidebar and rail
Keep only the connection dot (no pill/wifi text) across shell
Add theme toggle (Auto/Claro/Oscuro) to mobile header with text on sm+
Show user chip in topbar only on sm+ (hidden on portrait mobile)
Remove redundant sync/refresh button (SSE + pull-to-refresh cover it)
