[R1.5] Landmark promotion criteria / when mapping fails #5

Reviewer: consensus->landmark logic vague; how many sequential bistatic consistencies promote a landmark; when does it fail?
Disposition: ANSWERED DEEPLY by stage 2 (paper 2): mapping fails because ~89% of detections are phantoms + a 6.45 m bias; a filter cannot invent/correct. To add: state the promotion rule explicitly and tie the failure to the phantom mechanism.
