Resolve fuzzy entries whose msgid changed only in reST markup

Upstream changed markup without touching the prose, e.g. :func: -> :deco:
for decorators, :const:`None` -> ``None``, fully qualified role targets,
and renamed parameters (*secs* -> *seconds*). Port the same token change
into the existing msgstr and clear the fuzzy flag.

An entry is only changed when every difference between the old and new
msgid is a markup token (or whitespace), and each replaced token occurs
verbatim in the msgstr as many times as in the msgid. Entries with any
prose change are left for human review.
