Scanner: group the vizcode package by stage

The package was 35 flat modules, and every new feature added more. It now
has one subpackage per pipeline stage: scan/ (repository to structure.json,
with the language adapters and packs), map/ (assemble and check
code-map.json) and layout/ (positions). The versions and the helpers every
stage uses (ids, gitsrc, readme) stay at the top level.

Only the package moves: the commands SKILL.md runs keep their paths, and the
map format, scanner, schema and rules versions are unchanged, so existing
maps rescan incrementally as before. Imports in the commands, tests and
viewer tooling, the schema and CHANGES.md lookups, and the paths in
SKILL.md, format.md and DEVELOPMENT.md follow the move.
