Polish portals: fix revamped_plus logos + style rough forms

Replace the placeholder 'W' logo with a WiFi glyph on the Google/Discord
revamped_plus landing cards. Add proper input/button styling to Jet_Blue,
Starlink, and CoxWifi (all used framework class names with no CSS loaded,
leaving unstyled default inputs). All 24 portals now render cleanly at a
mobile viewport.
