docs: MCP servers are registered in ~/.claude.json, not settings.json (#321)

CLAUDE.md's "MCP Servers" section stated that every server "is registered
in ~/.claude/settings.json". Claude Code never loads MCP servers from
settings.json -- only from ~/.claude.json (user/local scope, written by
`claude mcp add`) or a project .mcp.json. The false claim had real
consequences on the maintainer machine:

- push-gate was declared only in the settings.json mcpServers block, so it
  never started; it has been re-registered with
  `claude mcp add --scope user push-gate`.
- figma-api's FIGMA_ACCESS_TOKEN lived only in that inert block, so the live
  figma-api server ran with no token; it was copied into ~/.claude.json.
- the 26-server inert block has been removed from ~/.claude/settings.json.

Changes in this commit:
- CLAUDE.md: the MCP Servers section now names ~/.claude.json and
  `claude mcp add --scope user`, and says a server declared only in
  settings.json never starts.
- CLAUDE.md, README.md, SRS.md: "Last Updated" / "Date" bumped from
  2026-08-07 / 2026-08-10 to 2026-09-13, to reflect the #320 standards
  removal and this fix (the bumps were already pending in the working tree).

No code changes.
