docs: consolidate Phase 2 documentation and reorganize docs folder

- Consolidated PHASE2_STATUS.md and PHASE2_COMPLETE.md into PHASE2_DOCUMENTATION.md
- Moved documentation files to docs/ folder for better organization
- Updated docs/index.md with new documentation structure
- Added quick links and key features summary to index
- Moved LMSTUDIO_SETUP.md to docs/ folder

Documentation now centralized in docs/ folder with comprehensive index.
